We consider mistake factual when content is not factually correct to its source.
Examples
NieR Automata is set in year 11854 (source) - is a factual mistake according to the given source
There is no YoRHa cycle because reason1(source) reason2(source) - you not aggreeing with the educated assumption based on correct sources is not a factual mistake.
