We consider mistake factual when content is not factually correct to its source.
Examples
NieR Automata is set in year 11954 (source) - is a factual mistake according to the given source
There is no YoRHa cycle because reason1(source) reason2(source) ... - you not aggreeing with this is not a factual mistake but an educated assumption based on sources.
